I understand that there is now a "sizeable" Form to be filled in when landing at Dalaman.

Good idea. They might need to contact you if someone falls ill and tests positive.

But is it outwith the wit of man that forms are provided by the airlines to filled in on the 4 and a half hour flight.

Also is most of the information not available from the airlines if required.
