Presumably those not connected to the internet will be buying their flights or holidays either via post, phone or from a travel agent. In this case I would expect their agent to advise on need for visa and, probably, provide one (with charge for the service).

Not that it would mean there will be of course.

Self service Turkish style, machine and a bloke to work it for you.

But seriously, with the visa being a "on demand" visa, without any data collected from the applicant then its likely there will be some provision on arrival.  Alternatively they may make it a condition of carriage for the airlines that passengers have a valid visa before being allowed to travel.  Can see some dopey check in punter trying to do the maths at 4.30am to see if you're within the 90/180 day rule!
