Do any of you guy's or gals connected with the medical profession, know about this form?
My Brother's Dr, told him he would get it from the specialist, the specialist's nurse, told him today to phone C.A.B they are very helpful....
I spoke with MacMillan charity today, they told me that either the Dr or the specialist would issue my brother with this form.....
I phoned the NHS helpline, they said they would make enquirys & phone me back within the hour.... 3 hours ago!!
Can anyone help with this info please, just trying to help my bro, Ta.
